2011 NHDOC Holiday Package Order Form

1. Be certain the Inmate Number and Facility are correct, complete and current. We cannot be responsible for non–delivery when these are not correct. Please be certain that the inmate is allowed to receive a holiday food package. Refunds will not be issued for delivery to an inmate that resides in a restricted unit.

2. If you have additional orders, please put them on a separate order form.

3. Even if you are ordering for yourself, please read the ordering guidelines carefully.

Methods of Payment: There are two ways to pay for your holiday package.

1.      Granite State Candy Shoppe. There are four (4) way to order through Granite State Candy Shoppe.

Payments accepted are: Cash, Money Order, Cashier’s Check, Personal Check or a Credit Card (Visa, Mastercard, American Express and Discover)

• Order Online at “www.nhstateprisonorder.com”

• Order by Telephone at 603-225-2591; phone order will only be accepted Monday Through Friday 10:00am - 5:00pm

• Order by mail send the order form and payment to the address below; or

• Order by visiting Granite State Candy Shoppe and placing your order in the store.  In Store Ordes will only be accepted Monday Through Friday 10:00am - 5:00pm

 

 

2. Inmates may submit a completed Cash Withdrawals Slip, along with their

completed Order Form to Rodney Roy, Director of Recreation, at the Concord Facility.

Payment: Cash Withdrawal Slips must be made out to: NHDOC Recreation Trust Fund

      THE CASH WITHDRAWL SLIP DOES NOT NEED UNIT TEAM APPROVAL

 

WARNING: If you do not have sufficient funds in your inmate account, your order will not be processed

 

Order Limits: All weights on the order form are in ounces. Each inmate is allowed one (1) order regardless of weight. The order may not exceed 320 ounces (20 pounds).  Only products listed on this order form or at www.nhstateprisonorder.com are allowed. If the Net Weight of your package exceeds 320 oz. a delay in processing your order may occur and the inmate may not receive all items ordered.

 

Please try to coordinate with friends and family to insure only 1 order per inmate is placed.

 

Shipping Charges: There is a $4.00 Handling/Processing Fee to be ADDED to each order.

 

Item Changes: Every effort has been made to source and accurately describe each item listed on this order form. However, manufacturers and distributors often change or run out of particular items and reserve the right to substitute a similar item of equal or greater value.

 

The order deadline is October 28, 2011
